Alterations in regional right ventricular shape in patients following tetralogy of Fallot repair involving a transannular Patch

چکیده
Background Patients who undergo surgical repair of Tetralogy of Fallot (TOF) often experience abnormal remodeling of the right ventricle (RV), notably with chamber dilatation and associated dysfunction. The ideal surgical technique is controversial, but RV outflow tract repair utilizing a transannular patch (TAP) is employed to overcome the RV outflow tract obstruction in many patients with TOF. We hypothesized that analysis of both RV volumes and curvature in the different regions of the right ventricle can unmask differences in RV remodeling patterns following repair using these two surgical approaches.

Background: Tetralogy of Fallot is the most common cyanotic congenital heart disease. The systolic and diastolic function in both ventricles is altered even after successful corrective surgery for this defect with a transannular patch. Pulmonary regurgitation, a common complication after this treatment, is usually well tolerated in childhood. The aim of this study was to assess the combined dia...
متن کاملEarly and Long Term Outcomes of Corrective Operations for Tetralogy of Fallot: An Experience of Two Centers
Objectives: Pulmonary valve insufficiency and right ventricular dysfunction may contribute to early and late morbidity and mortality after repair of Tetralogy of Fallot. Right ventricular dysfunction may be attributed to ventriculotomy incision, especially, when it is combined with a transannular patch as employed in the transventricular repair. Transatrial/transpulmonary approach without ventr...
متن کاملTetralogy of Fallot: transannular and right ventricular patching equally affect late functional status.
BACKGROUND In tetralogy of Fallot, transannular patching is suspected to be responsible for late right ventricular dilatation. METHODS AND RESULTS In our institution, 191 patients survived a tetralogy of Fallot repair between 1964 and 1984. Transannular patching was used in 99 patients (52%), patch closure of a right ventriculotomy in 35, and direct closure of a right ventriculotomy in 55. Tw...
متن کاملTetralogy of Fallot repair results in activation of the renin-angiotensin-aldosterone system.
Fluid retention is a common problem following transannular patch repair of tetralogy of Fallot. The present study was undertaken to evaluate whether humoral substances may contribute to this process. Patients undergoing tetralogy of Fallot repair using a transannular patch technique were compared to patients undergoing simple ventricular septal defect repair. Hormone levels were determined by r...
متن کاملRight ventricular Hemodynamic Alteration after Pulmonary Valve Replacement in Children with Congenital Heart Disease
Introduction: In patients who underwent surgery to repair Tetralogy of Fallot, right ventricular dilation from pulmonary regurgitation may be result in right ventricular failure, arrhythmias and cardiac arrest. Hence, pulmonary valve replacement may be necessary to reduce right ventricular volume overload. The aim of present study was to assess the effects of pulmonary valve replacement on rig...
